Advantage Plan $50 One Time Initial Set-up Fee per Member $50 Per Month $10 Per Visit

Family Plus $90 One Time Initial Set-up Fee per Family** $90 Per Month $10 Per Visit ** Family is defined as primary member, spouse and dependents 18 mos. to 18 years of age.

All services provided at our clinics are covered, including X-rays. Services include, but are not limited to:

• Flu Symptoms & Sore Throat

• Prescriptions
• Athlete’s Foot/Fungal Infection
• Minor Surgical Procedures
• Asthma/Breathing Treatments
• Cough & Congestion
• Allergic Reactions
• Allergies
• Migraines & Neck aches

Products not covered (i.e. crutches, injections, meds) will be provided at discounted rates. Some restrictions apply (see contract for details.)

Additional Information:

• Members are required to provide a checking account number for automatic payments each month.
• Cannot be used to meet any insurance deductible.
• Advantage Plan is a membership; it is not insurance and may only be used at our location.
• Advantage Plan cannot be used if you wish to bill your insurance for the care you receive at our office and cannot be used with any U.S. government plan, such as Medicaid or Medicare.
