All services provided at our clinics are covered, including X-rays.
Products not covered (i.e. crutches, injections, meds) will be provided at discounted rates. Some restrictions apply (see contract for details.)
Additional Information
Members are required to provide a checking account number for automatic payments each month.
Cannot be used to meet any insurance deductible.
Advantage Plan is a membership; it is not insurance and may only be used at our location.
Advantage Plan cannot be used if you wish to bill your insurance for the care you receive at our office and cannot be used with any U.S. government plan, such as Medicaid or Medicare.
